Hospice C.N.A. is part of a collaborative, interdisciplinary team that provides high-quality, person-centered end-of-life care and support, including those living at our Kaukauna and Green Bay campuses as well as those living in a 5-county territory. C.N.A. works collaboratively with patients, clinical teams, and family members to ensure physical comfort and personal care needs are met based on individualized RN assessments and patient-specific needs and goals. C.N.A. provides personal care to support the physical, psychosocial, and cognitive needs of those we serve.
