St. Paul Palliative Care and Hospice Services Administrator

As a result of internal restructuring, St. Paul Elder Services has a unique opportunity for an RN with hospice, palliative care, home health, and/or aging services leadership experience to join our team as the St. Paul Palliative Care and Hospice Services Administrator who is also committed to our core values of dignity, compassion, respect, hospitality, and stewardship.

Essential job responsibilities include:

  • Coordinates, develops, implements, monitors, and evaluates the programs and services provided by St. Paul Palliative Care and Hospice Services’ interdisciplinary team in accordance with current NHPCO best practices, state and federal conditions of participation, organizational policies, and guidelines that govern the program.
  • Serves as the primary liaison between St Paul Palliative Care and Hospice Services’ internal and external relationship development and collaborations.
  • Serves as the primary collaborator with the Hospice Medical Director and Community Advisory Board to ensure compliance and the best possible palliative-related care and support.
  • Provides oversight, reporting, review, and evaluation of all hospice-related reporting and submission of QAPI, HIS, CHAPS, and other compliance reporting.

RN required, and CHPN desired.
