The Hospice RN Case Manager is part of a collaborative, interdisciplinary team that provides high-quality, person-centered end-of-life services/support, including those living at our Kaukauna and Green Bay campuses as well as those living in a 5-county territory. RN Case Manager works collaboratively with patients, clinical teams, and family members to ensure physical comfort and symptom management based on individualized assessments and patient-specific needs and goals.
