Hospice RN Case Manager is a part of a collaborative, interdisciplinary team who provides high-quality, person-centered end-of-life services/support including those living at our Kaukauna and Green Bay campuses as well as those living in a 5county territory. RN Case Manager works collaboratively with patients, clinical teams, and family members to ensure physical comfort and symptom management based on individualized assessments and patient-specific needs and goals.

Candidates with experience in hospice, palliative, or aging service preferred and committed to embracing St. Paul Elder Services’ core values of dignity, compassion, respect, hospitality, and stewardship.
